Celine Dion covering ACDC's You Shook Me All Night Long. My ears are still bleeding a year later.
Anything by Richard Cheese, god awful. Tori Amos' cover of Smells like Teen Spirit was terrible. It's wierd sounding and good in some ways but too wierd for me System of a Down covering Blink 182's "Dammit". I'm really into punk covers, so for some good covers check out Me First and the Gimme Gimme's and Throwback(or Throwdown) does a fantastic Metal cover of "Baby Got Back". Also A New Found Glory has a whole album "From Screen to your Stereo" which is a bunch of movie covers, the Goonies theme is the best on there.
