What do the elders now keep in the way of records for judicial committees and do they still complete the forms noted in the “Pay Attention To Yourselves And The Flock” where it says “Using the forms provided by the Society, the branch office should be notified of the person's name, the Scriptural reason for the disfellowshipping, and the date of the action. ( S-77 and S-79 forms )” and do they still “Prepare a written summary of the case and should be prepared by the committee and put in a sealed envelope to be placed in the congregation file“. If not does anybody know what happens?

Sorry I should have said why I wanted to know... I am trying to avoid a disfellowshipping (Well all that that entails - Them making it hard for friends and family to speak to you) and have a plan. It needs to be in the UK preferably..
