I have been given some photocopies of this book by another researcher here (many thanks to them).
They look as if they have been taken with an overhead book camera as they have hands and fingers in them. I have cropped the pages down and made them into a PDF.

You will see that the Crux Simplex illustrated in it is the same illustration as the one that the NWT used in its appendix.
There is another copy of this book in PDF form, but it seems to be a smaller format and the illustrations are slightly different (as you can see below), so I thought this might be handy for people to have in their collections and it seemed appropriate for Easter.
