I believe it refers to the group of JWs who are aware the WTS is a false prophet and teach false doctrine, yet remain JWs for some reason.

The mental image comes to mind of a patient rendered paralyzed by the 1st part of the anesthesia but not being rendered unconscious by the 2nd part. Then being awake and aware through most of the operation but not able to speak or move.
