G'day Randy,
Thanks for posting the 'backside' of the form. As I commented on the other thread, a feature of the process, as shown by this form, is the lack of real forgiveness of sins, especially as shown toward a repentant individual.
Note two points on this form's instructions which demonstrate this are:
1. The Appeal Committee is to determine whether the individual was repentant at the time of the hearing (italics theirs).
Clearly the mechanics of the process are to take precedence over the Christian quality of love and forgiveness.2. "Reporting Reinstatements and Deaths": Notice that the copy of the form notifying this is to be retained in the congregation's file. Why? Because a re-instated person is not really forgiven. He/she must go through a period of restrictions, which of course is a procedure not found in scripture i.e. the WTS have added to Scripture. The mind boggles at the comment that a report of a disfellowshipped person's death is to be retained in the congregation's file. It seems they think their authority and claim on former members extends even beyond death. Incredible!

It was issues here in the UK in October.
Note the details regarding the appeal commitee. The form is to be registered under the APPEAL COMMITEES CONGREGATION. That means that under the Data Protection Act the document is "lost" and therefore not subject to the DPA Search unless you know SPECIFICALLY what documetn you are asking for.
