I've been meaning to build this survey for some time. Built on Simon's great survey template for "Apostates", this survey is especially for "Unbelieving Mates" or simply those who may be dating or falling in love with their Jehovah's Witness (JW).
If you are interested in my story, the link below will have to do for now:

Tell us a little about yourself and your family.

Did you have any background with the Witnesses before you found your JW?
How does your family feel about your relationship with a JW?
Do you have any status or influence at the Kingdom Hall?
When did you start investigating the Jehovah’s Witnesses seriously?
Where did you find information? Internet sites? Books?
How difficult or painful was the process of understanding the JW experience and their relationship with “Unbelievers”?
Was it a big dramatic revelation or a gradual realization?
Did you convince your JW to leave the Witnesses? What has that been like?
How were your family relations affected by your involvement with your JW?
Do you and your JW have extended friendships you both enjoy?
How long have you been together?
Is there anything you look forward to doing with our JW if they ever leave?
What are you most proud of achieving in your relationship?
Is there anything you appreciate about the Witness lifestyle?
Do you have any religious affiliation now? What do you believe?
How do you now feel about religion in general?
Do you feel any guilt celebrating xmas or birthdays or doing any other JW "no-no"s?
Have you attended any face-to-face meetups of ex-JWs?
How do you explain the JW experience to outsiders? Do you bother?
Do you feel animosity or pity toward current JWs?
How do you respond to witnesses when they call at your door?
Storm the barricades or tend to the wounded? (do you favor activism or support)
What do you think is the most effective approach to reaching people still in?
Do you think the WTS can or should be destroyed, will continue on as-is or grow / change?
How has your life been impacted by your involvement with your JW?
Are there things in your life you blame the WTS for?
How much of your time is still spent on JW related matters?
What do you think of the ex-JW community?
Do you see yourself still being associated with the ex-JW community in 5 or 10 years time?
What are your plans for the future?
What advice would you give to anyone who is in love with a JW?
What would you change in your life if you could go back and talk to yourself?
Do you have any regrets about your life with your JW?
Can we read your life-story anywhere? (links to online or books)
