WTS have their own Will forms for JW's

Has anyone seen the JW WILL FORM?
I was asked last year, when I was attending meetings by an elders' wife, if I had already done my "Will" & if I hadn't the elders would provide me with a blank WTS Will form for me.
At the moment I thought to myself, my life insurance is for my daughter not the WTS. So I never talked to the elders about it.
When I was reminded if I had gotten my 'will form' from the elders I simply told the sister I don't have any assets just bills. She then never reminded me again.

I have heard about the will form. It came out about the same time I quit going to the Kingdom Hall. As I remember, it did have something to do with leaving assets for the Society to sell or more than likely for the Congregation to sell and give the money to the Society. I am afraid I never got a chance to see one of these forms, so I am speaking merely by rumor.

I never got to see it either. It would be nice if someone had one and could scan it in.
