Medicine, religion clash at Bronx hospital: Patient's family members are JWs
by behemot ina bronx woman with internal bleeding was on the verge of death, and doctors wanted to save her with a transfusion.. the only problem was the incapacitated patient's family wouldn't allow it because they are jehovah's witnesses who believe her fate is in god's hands.. the clash of modern medicine and religion led doctors at st. barnabas hospital to rush into court for an emergency order - granted tuesday by judge faviola soto.. "the attending physician felt it was urgent that if she didn't get a transfusion, she would die," said st. barnabas spokesman steven clark.
"hopefully, the patient will be okay now.".
the patient, jean leslie, 55, is a stroke victim who had been living in the st. barnabas nursing home since 2008.. she was admitted to the hospital on may 21 when doctors noticed a problem with some blood tests, according to court papers.

I filled it out, but had an issue. The directions say to fill and mail back immediately. The media were all saying to fill out and return ASAP. On the other hand, it asks who is living at that location on April 1. Since I got my form weeks before the 1st, how do I answer? What if I'm in an accident and die on the 30th? I'm not a fortune teller!!

by darkl1ght3r in(proverbs 26:4-5)4 do not answer anyone stupid according to his foolishness, that you yourself also may not become equal to him.
5 answer someone stupid according to his foolishness, that he may not become someone wise in his own eyes.. so which is it?

I found this take on the versus from here
QUOTE:
In his article, "Is Proverbs in Contradiction on Answering Fools?" James Patrick Holding feels that the alleged contradiction between Proverbs 26:4-5wins a major award for silliness. What we have here is not contradiction, but dilemma—an indication that when it comes to answering fools, you cannot win—because they are fools, and there is no practical cure for foolery (as this citation demonstrates). So: It is unwise to argue with a fool at his own level and recognize his own foolish suppositions, but it is good sometimes to refute him soundly, lest his foolishness seem to be confirmed by your silence.
In his Alleged Contradictions in the Bible, B.J. Clarke points out that the close proximity of these verses (back to back) would rule out the idea of discrepancy even for the most sophomoric of scholars. James Jackson, in his article, "Answering the Fool,"suggests that "such close proximity reflects design, not disorder."

Sorry to brag, but when I finished 8th grade, I'm sure I would have passed this test. It wouldn't have been with flying colors (we were not taught how many bushels per cubic foot, for example, or Kansas-specific questions), but I would have passed, maybe as high as a B, B-. That being said, today I would fail. I have not consciously thought of the parts of speech, etc., I just use them!

It might have something to do with their new design rollout. This is blurb from Gizmodo:
