April im Austria Trend Parkhotel SchönbrunnWien (pts026/20.04.2017/12:25) - Wenn Anästhesisten, Notfallmediziner, Chirurgen und Blutbankmitarbeiter auf den "Wiener Bluttagen" Themen der Transfusionsmedizin erörtern, wird das Thema "Patient Blood Management (PBM)" für besondere Aufmerksamkeit sorgen. Aktueller Anlass ist eine soeben fertig gestellte EU-Richtlinie, die europaweit die baldige Implementierung eines strukturierten Programms zum bewussten Umgang mit menschlichem Blut empfiehlt.
"Diese Initiative war längst überfällig", sagt Oberarzt Dr. Peter Perger (FA für Anästhesiologie und Intensivmedizin). "Wir stehen vor der paradoxen Situation, dass mehr als 100 Maßnahmen für einen optimierten Umgang mit Blut wissenschaftlich gut erforscht sind, es bisher aber an einer strukturierten und flächendeckenden Umsetzung nach einheitlichen Richtlinien fehlt. Es gibt zahlreiche Einzelinitiativen, aber es fehlt eine umfassende, interdisziplinäre Gesamtstruktur."
Trotz Rückgang immer noch viele Transfusionen unnötig
Die Folgen: Zwar ist der generelle Trend zur Bluttransfusion seit Jahren rückläufig - so ist die Zahl der im Wiener KAV transfundierten Konserven seit 1999 um mehr als 40 Prozent gesunken - dennoch gehen Experten von weit höheren Einsparungspotentialen aus. So hat eine im Auftrag der European Society of Anaesthesiology (ESA) auch in Österreich durchgeführte Studie gezeigt, dass eine erhebliche Anzahl von Bluttransfusionen außerhalb der Behandlungsrichtlinien und ohne zwingende Not verabreicht wird. "Unnötige Transfusionen kosten nicht nur Geld und Ressourcen, sondern können für die Patienten auch gefährlich sein. Oft werden sie aus reinem Sicherheitsdenken durchgeführt, aber diese Sicherheit ist trügerisch", so Univ.-Prof. Dr. Jens Meier, einer der Studienautoren und Vorstand der Klinik für Anästhesiologie und Intensivmedizin am Kepler Universitätsklinikum Linz.
In vielen Fällen wie etwa in der Hämatoonkologie oder bei Patienten mit akuten Blutungen, Thalassämie oder Autoimmunerkrankungen sind Bluttransfusionen absolut notwendig und lebensrettend. Es wurde aber in zahlreichen Studien nachgewiesen, dass die nicht indizierte Gabe von Erythrozyten- oder Thrombozyten-Konzentraten das Ergebnis von Operationen und den Krankheitsverlauf negativ beeinflussen kann. "Eine Bluttransfusion ist letztlich nichts anders als eine Organtransplantation und sollte daher nur in zwingend nötigen Fällen durchgeführt werden", so Dr. Perger. "Trotz aller Sicherheitsstandards können Nebenwirkungen nie völlig ausgeschlossen werden."
Australien: 40 Prozent weniger Transfusionen - 60 Prozent weniger Sterberisiko
Dass ein bewusster und sparsamer Umgang mit Blut nicht nur Kosten, sondern auch Patientenleid spart, zeigt eine soeben veröffentlichte Untersuchung an 600.000 australischen Patienten. Dort wurde allen staatlichen Krankenhäusern vor acht Jahren ein strukturiertes Blood Management verordnet. Nach fünf Jahren ist nicht nur die Zahl der Transfusionen um 42 Prozent zurückgegangen, sondern auch die Wahrscheinlichkeit nach einer Operation zu sterben um 60 Prozent gesunken. "Das zeigt, welches Potential bei uns noch zu heben ist", so Prof. Meier. "Die Studie zeigt, dass die gezielte Vermeidung von Bluttransfusionen dazu führt, dass Patienten mit den bestmöglichen Voraussetzungen auf den OP-Tisch kommen. Es gibt in der Medizin nicht viele Maßnahmen, mit denen sich die Patientensicherheit derart verbessern lässt und die gleichzeitig auch noch Geld sparen."
In Österreich fehlt politischer Auftrag für Blood Management
Um Ähnliches auch in Österreich zu realisieren, wäre kein großer Aufwand nötig. "Wir brauchen jetzt den politischen Auftrag an die Länder, die als effizient erkannten Maßnahmen auch gezielt umzusetzen", so Dr. Perger. "Wie auch die eben veröffentlichte EU-Richtlinie nahe legt, bedarf es dazu in jedem Krankenhaus eines einschlägig geschulten Beauftragten, der in der Lage ist, Verbesserungspotentiale zu erkennen, und das Pouvoir hat, sie umzusetzen."
Blutarmut behandeln statt Blut zu transfundieren
Als eine der wichtigsten Maßnahmen erachten Experten die rechtzeitige Behandlung von Anämien. Bei etwa 20 bis 30 Prozent der Bevölkerung sorgt ein verminderter Hämoglobin-Gehalt oder zu niedriger Anteil von Erythrozyten im Blut nicht nur für ständige Müdigkeit, Schwindel, Herzschwäche, Kopfschmerzen oder Depressionen, sondern auch dafür, dass im OP-Fall eine Bluttransfusion nötig wird. "Durch den Trend zu veganer Kost, die älter werdende Bevölkerung und den zunehmenden Einsatz von blutverdünnenden Präparaten wird dieses Problem noch größer werden", so Dr. Perger.
Wird ein Eingriff nötig, haben solche Patienten schlechtere Chancen, ihn komplikationslos zu überstehen. "Wer eine Anämie hat, hat einfach weniger Reserven. Das ist als würde jemand eine Wüstendurchquerung mit halbleeren Tanks antreten", so Prof. Meier. Da die Bildung von genügend Eigenblut auch nach Eisen- oder EPO-Gaben im Regelfall vier bis sechs Wochen dauert, reichen die wenigen Tage, die Patienten vor einem Eingriff im Krankenhaus verbringen, oft nicht für eine optimale Vorbreitung. "Sinnvoll wäre die Einrichtung von zusätzlichen Anämie-/PBM-Ambulanzen, idealerweise innerhalb der Präanästhesie-Ambulanz integriert, in denen solche Patienten identifiziert und rechtzeitig mit vergleichsweise geringem Aufwand behandelt werden könnten", so Dr. Perger.
Auch niedergelassene Ärzte können das Blutbild verbessern helfen. "In 80 bis 90 Prozent lässt sich aus den jetzt schon erhobenen Laborbefunden herauslesen, dass und warum jemand blutarm ist", so Dr. Perger. "Das Problem ist, dass eine einfache, aber zeitaufwändige Eisentransfusion von den Krankenkassen nicht bezahlt wird. Diese würden ein Vielfaches ihrer Kosten wieder einsparen, weil die Patienten mehr Reserven für eine rasche Genesung haben und Komplikationen wie z.B. verlängerter Spitalsaufenthalt vermieden werden können."
Optimierte OP- und Labor-Technik hilft Blut sparen
Auch vor und während der OP gibt es zahlreiche Möglichkeiten, Blutungen zu verhindern und damit den Bedarf an fremdem Blut zu reduzieren. "Mit einem gezielten Gerinnungsmanagement und der Wahl der optimalen OP-Technik lassen sich viele Transfusionen vermeiden", so Prof. Meier.
Bewährt hat sich etwa auch der Einsatz von "Cell-Savern", die abgesaugtes Blut während der Operation filtern, säubern und die roten Blutkörperchen wieder in den Blutkreislauf des Patienten zurückführen. Ebenso lässt sich die Menge an abgenommenen Blut mit modernster Labortechnik stark reduzieren: Wo üblicherweise 15 Milliliter nötig sind, können Geräte der neuesten Generation bereits aus einem Milliliter alle notwendigen Werte herauslesen.
OP-Termin am Patienten statt an Sachzwängen ausrichten
Weiteres Optimierungspotential orten beide Experten in der flexibleren und auf den jeweiligen Patienten zugeschnittenen Planung des Operationstermins. "In der Praxis wird ein OP-Termin festgelegt und dann versucht, den Patienten bis dahin noch irgendwie zu optimieren", so Dr. Perger. "Wenn der Chirurg bereit ist, der Patient aber noch blutarm, wird transfundiert. Derzeit könnten 50 bis 70 Prozent der Patienten besser vorbereitet werden."
"Wir müssen das Bewusstsein für eine wirklich individuelle Risikoabwägung schärfen und in jedem Einzelfall entscheiden, ob eine frühere OP mit Transfusion oder die Verschiebung des Eingriffs das größere Risiko darstellt", sagt auch Prof. Meier: "Nur dann hat jeder Patient die Chance, mit den bestmöglichen Voraussetzungen ins Rennen zu gehen."

"Face the Facts", 1938 J.F.Rutherford ..... start listening at 41:00 speaking him against "totalitarian combine" or "monstorsity" and "the Roman Hierachy" (enemies of the people) who fight against Jehovah, who was called the "the friend of the people".
Did Rutherford exploit the people's fear of totalitarianism for his purpose or did he upbuild them with positive hopeful biblical thoughts inmidst a time of persecution ?

The Finished Mystery quote above was page 251 of the 1917 edition. Rather than stand fast to its beliefs and principles, Watchtower meekly followed the United States governmental ordered to remove pages 247 to 254 from The Finished Mystery.
In the 1918 edition, the section covering the sub-heading “Out of the mouth of the dragon” was reduced from 5 pages to 1 paragraph. Click here for a PDF scan of the pages from the 1917 edition that were removed from the 1918 edition.
Watchtower was not imprisoned for preaching truth about God’s Kingdom; it was for taking side in politics. Their release was equally a violation of their principles, a result of complying with the government order to change what they had printed. Neither action showed them to be spiritually cleansed.
