The ugliest I've seen was the one with the ugly guy/girl face with the ponytails, damn I can't remember his name, he had like missing teeth or something, that always made me jump when I saw it LOL! If any of you remember please post it. I don't think he's around here anymore. Daystar's is pretty ugly, he looks like a tormented demon.

OK, if you people won't accept my other answer, then my vote for second-ugliest avatar is Mastodon, just because his avatar freaks the hell out of me.
codeblue's avatar is a little unsettling, too, it hurts my brain.
