I believe some websites/pop-ups access the cookies that were created by other sites and these cookies may contain such info.

I wouldn't worry about it. It doesn't hurt to check the history though.

They may be able to do it by instaling spyware on your PC, it's always a good idea to have anti spyware to stop or erase such programs, ad-aware and spyware-blaster are good and free. Or better still don't use IE unless you really have to, use opera or firefox instead. Often downloaded free software programs contain spyware.

Sadly there is no one perfect program. Not one removes EVERYTHING (with regards to spyware and or anti-viruses)

WAC, that was a great list of useful anti intrusion software with all the links ready to be clicked.
All I have to add is that I use spyware doctor because webroot was eating up too many resources of my PC. Also I use Kerio as firewall:
