Do you seriously think Jehovah will be impressed by you having a piece of paper that says you'll do god's will whilst unconcious when you admit you don't bother doing whilst concious?

From memory you cannot GET a bloodcard, as you are not a JW or a minor child of a JW. You can carry a self-made document saying something like this;
"I, being a fearful person who has had their feelings of inadequancy and guilt played upon by religious cultists, think god is so stupid that even if I ignore his requirements (as told to me by aforementioned cult not that they can prove they are right) 24 hours a day, he will forgive me if I have someone else take responsibility for me by following written instructions when I am unconcious.
If I need a blood transfusion please ensure (to follow the religious cult's requirements) that it is fractionated and transfused into my body in its seperate components rather than as whole blood. I know that 'whole blood' being 'BAD' and all of bloods components put together being 'GOOD' is stupid, what do you expect when you have religious cultists make rules concerning medicine? The JW doctrine on blood is like saying buying a stolen car is bad, but buying all the parts of a stolen car and putting them back together is good, but despite this incandescent level of idiotcy I feel confident a piece of cardboard will keep me safe as I know that rabbit's-feet are supersticious nonsense."
Oh, Bee, by knowingly and willing associating with apostates (that's us dear) you are again 'sinning' in the eyes of the JW's.... you see they don't want people talking to ex Witnesses.
To use another car analogy, you are buying a used car (a religion) from someone who will tell you MUST NOT TALK TO ANYONE who had one of those cars and sold it, as they are evil liars.
Now, most people wouldn't buy a car from such a person as they would assume there was a problem with that type of car if the person selling it didn't want you to talk to ex-owners...
... now, take a deep breath and stop being so silly. You might actually benefiot from listening as you seem to have precious little in the way of a clue yourself.

If you truly fear blood, the "No Blood card" won't do it. You must clearly state your wishes in the type of document that blondie has patiently described to you. For further protection, file a copy of the document with your doctor. If you have visited any of the local hospitals, also advise them of your wishes in advance.

Just freaking Google and find the wording for medical advanced directives, print it out on a piece of card stock (which costs like 5 cents), and put it in your wallet. Then give it to your doctor for his file. You don't need us to research it for you.

The only way to get one is to be a "baptized" witness. They usually dont give them out to "unbaptized" publishers. Why not get the scan that jwfacts mentioned, print it out, get some adult friends to sign it as legal witness, nominate someone other than you mom as primary and secondary contacts and carry that with you?
keep it as uncomplicated as possible.
Trouble is if you are hospitalized in an emergency and they have to use it, the medics will call in the local hospital JW chaplain and he may then call in bigger guns, then because you are not as you say you are by the fake card, they may choose not to help you.
Couldn't you just wear an emergency bracelet, with the same instructions? NO BLOOD......?

You are clearly a retard, or you're a troll. Since you're not accepting help from ANYBODY, here is my suggestion:
Go to the medical store that is just down your street. Go to the customer service desk and ask for one. The vampire employees will all witness the signing of your no-blood card, and proceed to suck out your blood through your neck. If you have any trouble, just ask for Nosferatu.
After your card is signed and your body is drained, you will have a legal document stating that you do not have any blood.
