Often, how some one cares for themselves is a reflection of how they view life, how they like themselves, how much self control they have, etc. etc. I would hardly call these things shallow.

Placing too much importance on and being judgemental about how a person looks, cares for themselves or deciding they must not have self control, this is being overly concerned with the superficial.
I know a guy that drinks nothing but soft drinks, never drinks water, loves sweets, and won't eat vegetables, especially raw.
He doesn't exercise.
He's not at all overweight.
His parents are both slim people.
So, you think that caring about whether your spouse is likely to live for 20 years or 50 years more is superficial? Well, I guess we're all entitled to our opinion. I'm not saying that is the only consideration, but I am saying it is not shallow to take those things into consideration.
I want to add that if a person has a preference for the thinner or more "beautiful" people, then let them mingle with the thinner and more beautiful people.
Someone being thin or beautiful does not indicate that the person takes good care him/her self. I know plenty of slim people that have terrible habits.

And this is the reason you care about size? I hope so.
But, that's not a two way logical relationship. Saying that overweight indicates a poor lifestyle does not mean that being slim indicates a healthy lifestyle. Skinny people who don't take care of themselves, smoke or drink excessively, party too hard, etc. are just as unattractive as people who are overweight. That has nothing to do with being shallow.
