My understanding was that all that JW designation did was get one a visit from a chaplain, not that the chaplain had access to medical records. I would definitely clarify that for future reference (with any minister-there are gossips in every religion). HIPPA laws do not allow revelation of medical history to anyone but authorized personnel, and I wouldn't think a chaplain is authorized unless they are specifically let in. I don't think claiming you are of a particular religion means that any member of that religion can examine your records.
