Seems pretty standard to me. Nothing to shocking or out of the ordinary. Now if the form asked questions like "How many brothers have been reproved for masturbating" then I would think that was absurd.

Seems like they must lack common sense. Like
Society: "What counsel did you give to congregation?"
CO: "I told them to obey you, and fear you, and give you more money"
Society : "Good boy (pats head)"
