We faded 13 months ago and we feel we are ex-JW's never to return. We did a very sudden fade after the DC never to set foot in the KH again.

I would say I'm (a), (b) and (c) alltogether. After how many years does an inactive non-attending JW become an 'ex-JW', for example? Should have defined the options clearly.
