Definitly in a resturant in Old Dehli. I had Dehli Belly for a week, lost 16 pounds and wanted to die!
Fader and I have probably eaten there too. On the other hand knew some great places in Delhi to eat.

I would have to say an A & W in Aurora, Ontario in 1969, I was preparing a last minute late night burger. I had just scrubbed the grills and everything was clean, I was making this burger and when I flipped it, it fell on the dirty floor (I hadn't got to that chore yet) and so my boss picked it up and put it back on the grill and continued to cook and prepare it, and I looked at him and said you can't do that, I will make a fresh one, he said,"no I am tired and want to go home" , so I handed him the spatula and said its on your head not mine. And so he finished it and wrapped it and had the car hop give it to the costumer. What a pig, That floor was so dirty from oil grease dirt and grime. I told the owner of the place and eventually they gave me fewer hours and then no hours and told me I was through, because I can't keep my mouth shut.
Lady Lee a cockroach from a resturant in Montreal wow yuck. But I have one to tell, my friends and I were eating in a high class resturant in downtown Montreal, when our food arrived and we began eating three of us found shards of glass in our meal. Livid to say the least, my friend wasn't going to say a word, however I did, I stood up in a loud voice and said may I see the owner. Everyone in the resturant stopped and looked and were trying to figure out what was going on. The guy came to our table and my friend said there is glass in my food and I said yes and there is glass in my food. And the other said the same . Let me tell you we were not whispering this, but were rather loud. The owner prepared three more meals and gave us drinks and said at the end the food was on the house, we had to just pay for our alcholic drinks.
But we did indeed make a scene.
And to the other poster up above, yes I have had Flaggl and it is just that terrible medicine..
So this is was my adventure into the most unhealthest places to eat!!!!!

That depends on the definition of 'unhealthiest'.
Scariest: A chinese restaurant near my office served me an order of mongolian beef that, half way through eating, I found the mashed carapice of a beetle like insect in. In addition to throwing up from sheer disgust, I didn't eat a bite of food for nearly 4 days afterwards--I still have food related issues stemming from that experience.
Non-'healthy: My Grandmother's house. I don't think she ever baked anything unless it included butter, cream, or copious amounts of oil or sugar. Not that I ever turned down a meal Granny made--they were always delicious--no where else on earth could you eat your cereal with half and half, have the very best scrambled eggs in the world (corn oil AND butter in liberal amounts was the secret) and have toasted pound cake slices slathered in butter for breakfast. Granny lived to be 97, so it didn't seem to hurt her much. I always came home a pound or two heavier after extended visits.

A Wendy's restuarant in NEPA. I ate there and about five minutes later had a severe allergic reaction to something in their food -- a reaction I'd never had before. So, I figure it was just unsanitary cooking conditions that caused the reaction. Needless to say I'll never be eating there again!!!
Also when I was a teenager I worked at a high-class restuarant, formal dining, which will have to remain un-named, and they reused partially eaten bread, butter and salad dressing left over by the customers. It was quite disgusting!
There's a Mexican/Irish restaurant near my home. Yes, Mexican and Irish, together on the same plate. The first time we went there, it tasted ok, but then I got food poisoning. Then, for some reason, WE WENT BACK numerous times. To their credit, on subsequent visits, I only got indigestion.
Another nasty place to eat is Applebee's, a big chain in the U.S., also KFC in my town is very dirty. We used to have a Denny's, but they had too many cases of food poisoning and got shut down, finally.
Over the years KFC has really gone down in quality. Once in Niagara Falls NY side we stopped after driving 10 hours to eat and same as before, ahhhhhh ice cream........
