When he came to the hospital, he did not look well.
You might think that everyone with a recent leukemia diagnosis appears ill, but that isn’t true. Some go to their primary care doctor with vague complaints of feeling tired or of having a cold, and a few hours after having their blood drawn are stunned to receive a phone call that they must get to the hospital immediately. Some get “stuck” in the Kübler-Ross stage of denial, refusing to believe they have leukemia even after they’ve received chemotherapy for it, or in the bargaining stage, negotiating for just one more bone marrow biopsy to confirm the diagnosis.
Others look like my new patient.
“His hemoglobin is 2.7,” my leukemia fellow told me. A normal hemoglobin for a man should be closer to 15. “When we drew his blood, it came out almost clear.”
“How many bags of blood are you going to give him?” I asked.
She hesitated. “He’s refusing any blood product transfusions. He’s a Jehovah’s Witness.”
The Jehovah’s Witnesses are a religious group that believes, among other things, that only a minority of people reach heaven. They also believe that it is wrong to receive the tissue of another human being, and that doing so violates God’s law, even if potentially lifesaving.
This presented quite an ethical quandary. To treat our patient’s cancer adequately, we would need to give him chemotherapy at doses high enough to obliterate his entire bone marrow, at least for a while. During this time, he would not be able to manufacture his own red blood cells or platelets, so we would have to support him with transfusions — sometimes as frequently as daily. Supportive measures like blood product transfusions and antibiotics have led to a marked drop in mortality from the actual leukemia treatment over the past half century, to less than 5 percent. Without such supportive measures, death is almost guaranteed. Without chemotherapy, though, death is certain, at the hands of the leukemia itself.
My fellow and I walked into our patient’s room to talk with him. He was lying in bed and surrounded by his relatives, who were sitting in chairs or on his bed. He smiled weakly at us, pale but handsome. We introduced ourselves to everyone and then started talking about how to treat his leukemia.
“So, you understand that the therapy we recommend has the potential to cure you, but it is unlikely you’ll be able to survive the chemotherapy without receiving blood transfusions?” I summarized.
“I understand,” he said.
“But if we don’t treat your leukemia, you will die from it.” I looked him in the eyes, at the crow’s-feet that told me he had laughed a lot of times in his life, and down to his hands, holding those of his niece, sitting by his side. Tears were streaming down her cheeks.
“I know.”
“I heard that you’re a Jehovah’s Witness. I come from a different background – I’m Jewish.”
“Well, nobody’s perfect!” his brother piped up. Everyone laughed, easing the tension in the room.
“I have different beliefs from you, so I can’t say I totally understand where you’re coming from, but I respect your faith, and we’ll follow your lead on what you want to do,” I told the patient. “You’re the boss.”
“No, he’s the boss,” he said, pointing toward the ceiling, and beyond. Others in the room nodded in agreement.
He declined transfusions, but he did want to receive chemotherapy, so we compromised on a lower-dose treatment, and minimized our lab draws to one per week, to preserve what blood he had. After two weeks his hemoglobin dropped to 2.1; after three weeks, it fell to 1.8. One month into his hospitalization, he became too dizzy to stand, and his heart, starved for the oxygen his missing red blood cells would have carried, just stopped beating. He had cardiac arrest and died.
The staff on the leukemia floor took his death hard, all plagued by the same question: Had we done enough for him?
We enter our chosen profession to kill that malignant golem, leukemia. But our patient approached his leukemia focusing more on eternity than his time on earth. While many felt his belief had tied our hands so that we couldn’t treat his leukemia optimally, ultimately our responsibility must be to our patient’s goals more than to our own: in this case, his relationship to God, and his desire to reach heaven. I hope he made it.

¦ Severe anemia in a patient who will not accept blood products presents a significant challenge for the health care team. As health care providers, we must be aware of these patients' wishes and have plans in place to provide care from the time of admission to the day of discharge.
¦ Jehovah's Witnesses will refuse all major blood fractions, including whole blood, packed RBCs, plasma, and platelets. Minor fractions are allowed and the decision to accept or refuse these fractions is a personal one made by each Witness.
¦ The goals of anemia management include cessation of blood loss, meticulous conservation of blood, optimizing hematopoiesis, and assessing the patient's physiologic tolerance of anemia.
¦ Profound anemia with a hemoglobin level of 2.6 g/dL is survivable without a blood transfusion.
¦ A conservative dosing regimen of erythropoiesis-stimulating agents can be effective in increasing the hemoglobin level of severely anemic patients.

Police are appealing for witnesses after the Jehovah's Witness Kingdom Hall, Flansham Lane in Felpham was subjected to religiously aggravated criminal damage overnight between Thursday 11 April and Friday 12 April. Parts of the wall of the building were splattered with red, white and black paint and some of the graffiti included swastikas and the word 'CULT'. Detective Constable Ellie Hughes from West Sussex CID said: "Sussex Police take criminal damage of this nature very seriously and we are determined to bring to justice the person(S) responsible for such a mindless act. "Officers have speaking to the local community and I would encourage anyone with information no matter how relevant they think it is, to please contact Sussex Police on 101 quoting serial 401 of 12/04 or alternatively call Crimestoppers anonymously 0800 555 111." A 45-year-old man from Littlehampton has been arrested on suspicion of religiously aggravated criminal damage and has been bailed to 29 May pending further enquiries.

The WTBTS of Britain is producing books and magazines (23 million items for the UK) and are then selling these to the IBSA. For the year ending August 2012 this was at a sales cost of £11.7 million.
Excess funds from both the WTBTS of Britain and the IBSA are been donated (grant) to the WTBTS of Pennsylvania, who are then lending the money back as interest free loans for building purchases, etc. This is coordinated by the Governing Body of Jehovah's Witnesses.
The IBSA is donating its property to other WTBTS corporations. For the year ending August 2012 this amounted to £9.5 million.
The IBSA received £6.7 million in "grants" (donations) from a WTBTS charity in Germany.
Interestingly the grants received by the IBSA from the WTBTS charity in Germany over the past 3 years, where the European distribution centre is for the WTBTS, is equal to the claimed value of the books and magazines "donated" as charity aid to Europe by the IBSA.
Simply put, the WTBTS of Britain is printing books and magazines and are selling these to the IBSA. The IBSA then "donate" these items to congregations in Britain and to the WTBTS European distribution centre in Germany and claim the money value as "donations and overseas aid". The WTBTS charity in Germany, after receiving donations from across Europe, can then donates grants to the IBSA equal to the amount the IBSA donated in books and magazines.
Profits are donated via the WTBTS of Britain to the WTBTS on Pennsylvania, and the central fund operated which contains monies from the WTBTS of NY and Christian Congregation of JW's Inc. The WTBTS of Britain is registered for the secondary purpose of "funding the construction of places of woship in various countries" one of these being the United States of America.

by jwleaks injehovah's witnesses say it is not their responsibility to report child sex abuse to police unless mandatory reporting legislation is in place.. a victorian inquiry on thursday heard that when a jehovah's witness leader hears allegations of abuse they are directed immediately to contact the society's legal department to check their obligations under state law.. jehovah's witness legal counsel rachel van witsen said elders were not expected to know their legal obligations.. "this is done to ensure elders fully comply with any legal requirements that may be applicable in the state in which they reside," she said.. "in victoria if there was mandatory reporting the immediate advice would be to report that immediately.".
the society's director terrence o'brien said the decision to take the allegations to police lay with the victim.. "without mandatory reporting we don't feel that as ministers of religion that's our obligation to do that," mr o'brien said.. however, the organisation would encourage the victim to report it and would co-operate fully with police, he said.. ms van witsen said elders would make an exception if they felt the child was threatened.. "we would report that to appropriate authorities," she told the inquiry.. "we're about stopping child abuse.".

Salvation Army forced to defend handling of child sex complaints

The Salvation Army has had 473 complaints about abuse in its Victorian children's homes, has spent nearly $20 million so far settling them and could give no explanation of how predators got away with it for so long - but it denied there had been a culture of abuse.
The Salvation Army had almost no records of children in the homes but had not tried to hide, shred or dispose of them, Captain Malcolm Roberts told the Victorian inquiry into how the church handled child sexual abuse...
The committee also grilled Jehovah's Witnesses Terrence O'Brien and Rachel van Witsen about when the church might refer child sexual abuse to police.
Mr O'Brien, the director of the society, said if reporting was made mandatory the church would comply.
He said if abuse was reported to elders, they had to inform the church's legal section before anything else. It was the victim or family's choice whether to go to police.
