Cappy, a suggestion, since you're conceptualizing the site:
Why not make a simple questionnaire in the beginning and then that information would be automatically incorporated into the letter, that could then be printed?
For example:
a) Are you a baptized Jehovah's Witness? Y/N
if a) = Yes
b) How many years have passed since you were first baptized as a Jehovah's Witness?
c) Are you still an active Jehovah's Witness? Y/N
d) Have you currently got any special service privileges within the congregation or the Organization? Y/N
e) On what country do you live in? (Pick from a list of countries)
f) Do you want to remain anonymous when you send this letter? Y/N
if f) = yes
g) You must choose an alias to use as signature. _______________________
etc
In the end, one can print the letter directly from the website, even with a choice of different letter types for variety. And it can be done in multiple languages.

What happened to "8/8 Dissociate"?
Also, what does 8/8 represent besides a catchy phrase? Also, we must remember, what works in english doesn't work in other languages. Doesn't work in a few that I can think of. It must be as universal as possible and should actually MEAN something.
(Sorry, freemindfade, not trying to be difficult here, just contributing to the idea pool by playing the devil's advocate)

When I created the name and logo for my company and registered the brand name and trademark in my tiny country, I received several threatening emails from a big international law firm representing a corporation in the US intimidating me to desist from it or else be sued. I didn't backed up and the local trademark office upheld my case. I won, and they had to pay all legal fees. I then offered to sell them the brand and trademark for a ridiculous amount of money, since they were so eager to protect their brand. They never replied ever since.

The letter type isn't the same, the lack of the dot doesn't imply the .org brand, and the blue background is a different blue. What's to complaint about ? ;)

Raymond Franz was disfellowshipped on a November 25th. Why not "JW Insurgency 25/11" ?
Perhaps add a footnote: "Raymond Franz was a former member of the Governing Body of the Jehovah's Witnesses who was disfellowshipped on November, 25h,1981, for the sole 'sin' of holding a different opinion than that of his religious peers. In memory of his plight, we choose to associate this date to our movement."

I understand your concern, but there are ways around it.
For example, a Circuit Overseer is simultaneously an Elder, and an Elder is simultaneously a publisher. If you're a CO and you're concerned that the Service Department might narrow down on you too dangerously close, just pick a broader status. Or simply do what THEY normally do: use "theocratic warfare".
What I think is vital is that ONE LETTER = ONE REAL PERSON. No letters in the name of others, and no making of letters of fake people just to inflate numbers.
Anyway, here's another suggestion for the intro paragraph that leaves things more vague:
"I’ve been a baptized Jehovah’s Witness for over 99 years. I’m currently an active / inactive publisher with / without special service privileges, assigned to a congregation in state / country."

And here's my suggestion for an intro of the letter:
"To whom it may concern:

I’ve been a baptized Jehovah’s Witness for over 99 years. I’m currently an active / an inactive Publisher / Regular Pioneer / Ministerial Servant / Elder / Circuit Overseer / Bethelite, assigned to a congregation in state / country.

This letter is intended to let you know that I no longer wish to be identified as one of Jehovah’s Witnesses, and therefore I wish to be dissociated from this congregation and its legal entity, the Watchtower Bible and Tract Society, as well as any of its branches and brands. I can’t tolerate any longer the embarrassment and the mental suffering that comes from being associated with this harmful cult, and these are my reasons: "
