    LittleToe,

I'm probably one of the few people who got to fill in his own

I'd think you must be the only person who's got to do that.

I hope you made it memorable! Did you keep a copy? Wanna post it?

    Crumpet,

they only have one line for disfellowshipping offences!

Don't worry, there's a whole session in elder's school teaching you to write small...

"Specify by what action(s) the person disassociated himself":

Mr. Evans called Brooklyn, Bethel and informed the brother in the Service Department who took his call that everyone in responsible positions at Bethel were apostates due to our acquiring a pass to enter the United Nations building in order for the brothers to research in their library. Mr. Evans then called his Presiding Overseer and another elder via telephone conference and informed the brothers that the WTBTS is a harlot for 'joining' the United Nations as a Non Government Organisation and he resigned from Jehovah's Organisation.

Recommendation: Let him die at Armageddon which we know is imminent because Jebus returned in 1843 1874 1914.... well, you get the picture.

Okay I've tried it and you cannot fit in the following no matter how small you write:

fornicating, drinking, loose conduct, drug abuse, smoking, murdering, greed, blaspheming, disobeying parents, idolatry, lying, thief ( i hasten to add that I only stole a token thing so I could add stealing to my list of things to report to the elders at my first judicial and do not consider myself a thief - but the rest still stand!)

    (only kidding)

Not surprisingly, they get donations from the congregations in a specially-colored envelope, too -- GREEN! (Seriously)

Is this something new? When I was account servant I mailed donations directory to the society along with the proper forms. I used ordinary white envelopes I bought at Staples.

Hmmmm.... Good question. I remember sending them in in green envelopes (I was the no-count servant for a few months in a small congregation) and I remember the service overseer telling me, "Remember, it's green for money." Maybe that was just this congregation? Or maybe it's one of things that only matters a little bit?

(I'm guessing if you sent them money in a black envelope with a glowing pentagram on the front that oozed ectoplasm, they'd still find it in their hearts to accept the cash.)

No, there are 5 MS in my family, 5 congregations, 2 states, all used green envelopes. It helps them find them easily in the large volume of mail that comes into Bethel everyday (mostly in white envelopes).

I like the very last line on the form...." DO NOT KEEP A COPY OF THIS FORM"

Sounds like someones a little afraid of legal actions!
