Alamb, perhaps you could make a separate thread with your request. More people would see it. I seem to recall a case from a member of this board, so perhaps he/she will respond. I can't remember who it was, though.

I found someone closeby who can put in into digital format as soon as tomorrow. I hope to have it up by tomorrow evening. Thanks for the links.
