I can't help you. When my wife wasn't looking, I threw all of ours into the garbage can.
I had completely forgotten about those. I remember now tearing mine up and burning it. This was my mini ceremonial parting of ways with the J-bots.

I always have and always will carry a blood card. Its just that now mine has a Red Cross on it, and my blood type listed.

Is this the current card in the UK too?
The current 'new card' in the UK is a large A4 sheet that has to be folded, origami style' down to the size of the old card that fitted in the wallet.
In it you can give away your rights to medical privacy to the local Elder window washer, and toilet cleaner, salesman, blah blah, at your Kingdom Hall. They can tell their wives, and their wives will then tell everyone else in the KH! Your piles will NEVER be your own private business again.
Whether they've stooped so low in the US remains to be seen. Hopefully, some helpful Bethelite will scan us the US NO BLOOD card. If not, all will be revealed soon enough! I doubt whether it would be legal in the US....but you never know.
That is not the current card.
The current card in the US came out in January 2005. It does not request the privilege of viewing your medical records.
There was no new card for '06 or '07. The 2005 card is good indefinitely, or until the jw changes his conscience on something he filled out.
I'll scan one in when I get a chance if someone else hasn't done it already. although I'm sure there is a current one on the board somewhere.
