As far as its ability to almost convey that it was real, the first "Silence Of The Lambs".
It was unnerving to me.
Jacob's Ladder, was really bizarre as well.
You almost swear it's real, and that is what was scary.

I remember being in N. Dakota or S. Dakota laying on the roll-out bed, trying to sleep. My sister Sherry who was 13 kept elbowing me to make me watch this Vampire Movie. Not sure who was in it but it scared the bejesus out of me. That and my oldest sis's passion Dark Shadows.

I found "silence of the lambs" very disturbing and thought about it for days after seeing the movie..
I agree,"the shinning" is VERY SCARY.
Rosemerrys' baby was scary too.
I too saw the exorcist(sp) many years after the fact. it still scared me.
Oh yes, the amytivile horror " was very scary.

It isn't scary now, but Nightmare on Elm street scared the bejeebies out of me when it first came out.

THE scariest movie I've ever seen was... "The Night of the Living Dead." Ooooohhh! Talk about your heebee-jeebies! I had nightmares for WEEKS (I was about 7 years old when I saw it!)
After that, "Children Shouldn't Play With Dead Things." "Alien" also seemed sp freaky to me (as was "Predator"), that I had to see "II" and "III" before I could see the first one.
"The Exorcist"... was funny... at the time... due to the urination scene: my quick wit brother had us all laughing through the entire film after that point. But no way I can watch it now; I tried. Not that the contents scare me; I wouldn't know to remember. But I do know that the music makes my skin crawl (I think the crucifix scene did me in...)
"Dark Shadows"... was the BOMB! Go, Barnabas, Quentin and Angelique!
Peace, and... "pleasant dreams" (BRRRUUAAAAAHHHHHHahahahahahah!!)

Of recent movies, i found "Jeepers Creepers," "The Ring," and "Ghost Ship" to be pretty creepy.
I like movies that creep me out.
I know it is BS - it requires voluntary suspension of disbelief - like going to the Watchtower Study.
Want a real-life creeping out? Read about San Francisco's ZODIAC killer or the NYC psycho named Albert Fish. You've been warned...
