Clowns. And people dressed up as furry Disney characters. I think it has to do with not seeing the face and eyes.
I freeze, have a deep sense of dread, and could possibly react violently. -

Spiders is my worst. SFJ's pic made me jump out of my skin! There's a spider just over there....arrrrgh!
Also, needles, and blood. Mainly just any kind of skin puncture. *shudder* There are other minor ones, but those are the major ones.

The boogieman who lives under my basement stairs. Other than that, nothing!

The DEMONZ!
I hate being alone. I get edgy.
When I'm alone in the woods, I get freaked out beyond all belief. I tend to panic over things like snakes, animals. I can't breathe, I think the world has ended and I'm the only one left and I will wander the woods for all eternity until something kills me.
I'm just really glad I never got sent to Vietnam.
I think that the fear I'm describing is the origin of the word "Panic" because Pan was the god of lonely places and of the wilderness within the soul of man - and the Greeks called my fear panic.

1. That my daughter dies before me....terrible nightmare
2. Spiders....ewwww...I held a tarantula for about 5 minutes one time but that didn't help. I wanted to kill it the whole time.

Dentists; didn't get over it till too late to save my adult teeth.
Also, unemployment (I mean beyond the normal worry: fear of instant death is what it amounts to), venomous bugs & animals generally, and any carnival ride faster than a ferris wheel. Bees (I CANNOT "just keep still" around bees like everybody tells you to. I can't trust them to just lose interest and wander away).
Borderline phobias: Needles. Ants, but only in large quantities. Getting lost -- but I manage it rather than conquer it by NEVER leaving for a new destination without directions. NEVER.
Anti-phobias -- things I like that other people are phobic about: Snakes (non-venomous), spiders (ditto: including tarantulas), bugs other than roaches or swarming bugs. Mice - not that I approve of their damn teeth prints in the butter.
