Include his full name, DOB, and any other identifying numbers and dates.
I have all this also
With your written authorization to the police force there, they will provide you a copy of the report. Send it certified mail (not required, but helpful). Some departments have specific forms required to receive records. You can call ahead to the department and ask if they require a specific form.
I will try this and see if I can get anything.

Sorry to hijack the thread, but I have to laugh at the idea of anyone who advocates Jehovah's Witness beliefs representing his/her self as a "scholar." Jehovah's Witnesses are the antithesis of scholars.
