Absolutely definitely New York - but you will need more than 3 days.
Rome is a must - It's full of history and beautiful people, they certainly are the best dressers in the world.
London for it's fantastic blend of history and the ultra modern, it's got the best of everything including cuisine.
Cape Town - fantastic location and stunning views from Table Mountain and some fantastic drives to the winelands and the Cape peninsula.

Don't get stuck in capitals. My personal favourite is Florence (Firenze) -- when you're there you can easily go to Sienna, Pisa, Perugia and Assisi. And there is Venice of course.
Toledo, Granada, Córdoba, Sevilla.
Vienna, Salzburg.
Preferably not in summer when all those places are overcrowded. I was in Prague last week, it's a wonderful city but I'd like to go back in winter.
