My experience with the “blood issue” is this.
Not me but a relative that ended up dying in the hospital.
The HLC and elders in the congregation weren’t there for us. Did not even want to go to the hospital to visit an active publisher.
We are not that active, therefore not that important in the congregation. Our group overseer insisted we go to his place so he could tell us we should not have the funeral in the Kingdom Hall while before the patient even died.

Practices no doubt vary in different places, but from my own limited experience, over the past 20 years or so, elders seems to be very laid back over blood issue, they tend to respect privacy, and only get involved if invited by the JW patient to do so. This is a far cry from what I read in the old literature about health care workers leaking private medical records of JWs who broke the blood ban and parents kidnapping children from the hospital. I think those days are gone.

I’ll bet they got in serious trouble for that shit, their lawyers advised them that they were gonna lose badly if they pressed the issue in court, and their charity status was threatened if they didn’t back the fuck off.

It's probably in the heyday of those kinds of shenanigans that first got governments to seriously begin looking at the WT and the way it treats its people. Agitating in the name of pacifism is one thing, but bragging about the kids who've died in obedience to their doctrine* is a whole 'nother ballgame. Doctors having to wake judges up in the middle of the night to get a court order to have a child transfused became such a meme that there's even a Jack Chick comic about it. The GB probably began to feel uncomfortable about the amount of pushback they were receiving** and have gradually been quietening and watering down their stance.
* In a 1990s Awake!
** Kids dying who might have otherwise been saved isn't as glamorous as them sending their young men to prison over Vietnam.

I am sure that the official line is that the HLC are not there to force anyone…… but all the HLC reps are experienced elders who are themselves committed to ‘ no blood’ . The mere fact that they are there , giving spiritual support and encouragement is a powerful reason to keep the patient committed to the ‘right’ thing to do.
