1) Yes.
2) I was baptized at the age of 15.
3) I began to "fade out" around the age of 24

Got Baptized at age 16
Stopped going at age 22

3) Was disfellowshipped only a few weeks after I turned 25.

I was. My grandfather had studied with the witnesses back in the 1930s, so I'm a 3rd gen dub.

Nope. I'm thankful that my dad never pushed me to do it. He said I would know when the time was right & when I was ready to make the commitment. And I was never ready.

I bailed pretty soon after I turned 18.

I did not feel that I should be held to a baptizm at age 14. I did not know what I was doing. How can a child be held to a contract anyway?

That is ome of the big problems with "free will." JW's, like many evangelicals that hold to "free will" believe in some kind of magical and non-Biblical "age of accountability". At this age you, all of a sudden, realize that you are a sinner in need of a savior.
Don't feel bad, however. I asked my dad the same question before I faded. His response was, "You knew it was 'the truth' then, and deep inside, you still know it." Oy, is he in denial.
