MJ, I will check out the route 66 link, it sounds interesting and similar to the Roman roads here. The Romans starting in 43 AD built a superb quality road network across Britain which still exists in part today. Some roads still follow the same routes which were built in straight lines. In some places the flag stones still lie beneath the tarmac. In other places the road deviates from the original course and the old paths can be followed or the flagstones may be just below the surface.

If you drive out to the Grand Canyon, take time to see the Meteor Crater. It's just a few miles off the freeway, and an interesting place to take a break. Stand on a corner in Winslow, Arizona Also, there are Indian dwellings near Flagstaff that are interesting: Oak Creek CAnyon and Wupatki National Monument. (You can skip these if you're going to Mesa Verde in Colorado.) You can drive through Wupatki on your way to the East entrance to the Grand Canyon, where the car lines to get in will be shorter. There's also an Native American Trading post on HWY 64 on the way to the East Entrance where you can get all sorts of things that are made by craftsman on the reservation, including some very nice Turqoise jewelry.
To avoid crowds, try to schedule the Grand Canyon mid-week rather than on weekends, assuming that you're going to the South side. The canyon is busiest between 10 and 4, and goes quiet right after sunset. If you're going to the North side, crowds aren't a problem.
Since you're allowing 3 weeks for California, try to see some redwoods - either at Yosemite or Muir Woods just north of SF. There are more redwoods north of SF, if you're driving that far. I'd also suggest seeing Lake Tahoe - it's truly spectacular.
I also second the suggestion to see the Oregon coast and Washington (Mt. St. Helen's, the HOH rain forest, and Seattle.)

I understand when driving inter-state people use all lanes to pass on the inside and outside. Is it still courtesy to use one side as a slow lane and one as a fast lane?
Yes. Generally the left and center lanes are higher-speed lanes than the far right lane.
Do you use those two pin mains plugs in every state (110v) ? Does anyone know if they use the same power supply in Toronto?
I'm not sure about every state but here we use them
I don't know much about cell phones, try using a phone card instead. I personally don't think cells are too reliable.

I can't. A Brit cannot get a hire car in NYC, I don't know about other nationalities. I think we just drive completely differently and it would throw everyone into confusion. I can't remember the last accident I had, probably coming off a motorbike about 10 years ago, but we can't get insurance there.

Verizon has the best network and I think they have prepaid service.
Unless you know people in Dallas, I don't know what you would do here for 5 days.
Some rental car companies can be tricky, so make sure you can take the car out of state and into Canada and that you have unlimited mileage.

Yosemite Park is incredibly beautiful. I've hiked in there three times. You can do the brutal trail up to the top of Half Dome and back in one day if you're a strong bastard and don't mind walking bowl-legged for a few days after. I recommend you take the mule up and hike down though. Just being there is awesome.

Head to Napa Valley and enjoy the wineries. I liked Francis Ford Coppola's vineyard. Sample the wines and enjoy the beautiful weather.
